Form 10D is a pension withdrawal form, that needs to be completed post-retirement. It is used to sanction the amount saved an employee has saved up under the EPS.
If you have PF deposited under EPFO then you’re registered for both the Employee Provident Fund (EPF) and Employee Pension Scheme (EPS) but you must complete the KYC process to apply for pension. The member or their nominee has to fill in and submit the form through the employer to the EPFO.
Form 10D can only be submitted after retirement at 58 years old or after an early retirement at 50.
The compensation of your pension depends on the duration of your employment, your monthly salary, and when you retire. If you retire before the age of 50, your pension will be reduced by 4 percent every year you are younger than 50.

8(a). In the case of reduced pension
This part needs to be filled in if yor applying for pension before the age of 50. You need to mention the date, month, and year of the day of commencement of pension.
9. Option of Commutaion of 1/3 of Pension
Retired employees can claim their entire pension amount at once. They can either get the total capital amount or receive 30% of the amount and leave the rest for the monthly pension.
This field needs a simple yes/no. If you want a lower commutation, you can specify the percentage in the adjoining space.
10.Option for Return of Capital
Fill in this field if you want to withdraw your entire pension amount at once.
11.Nominee Details for Return of Capital
If you have checked ‘yes’ for the return of capital field, you need to fill in the following details:

This part should include the details of the employee’s dependent family. In the table, fill in:

Mention the date of death of the employee in who’s name the EPF account was registered. This field can be filled in if a nominee is claiming pension.

This part should include the details of the bank account the pension amount will be received in. Mention the following details:
